• Fill in the form and save the entered data
• Send the duly completed form to our Customer Service per e-mail: sales[at]rudolf-med.com .

After receiving the completed form from you, we shall send you an answer with an RMA Number (Return Material Authorisatzin) as soon as possible.

As the RMA Number serves to identfy, quickly process and trace back your reshipment, please write the RMA Number clearly visible on your reshipment packaging.
